Freight & Shipping Terms
Fortismp.com offers product samples to help customers evaluate Fortis Medical Products ostomy and self-care products before making a purchase through an authorized retailer or supplier.
A small shipping and handling fee may apply to sample orders. Any applicable shipping and handling charges will be clearly displayed at checkout before an order is submitted.
Sample orders are shipped using the shipping method available at checkout. Expedited shipping may not be available for sample orders.
Shipping Errors/Damaged Shipments
Fortis Medical Products carefully prepares sample orders for shipment. If your sample order arrives damaged, is missing an item, or contains an incorrect product, please contact Fortis Medical Products Customer Care as soon as possible after delivery.
Fortis Medical Products may request photographs of the shipping package, shipping label, and affected product to assist with the investigation.
When a sample is damaged during shipment, shipped incorrectly, or otherwise cannot be used as intended, Fortis Medical Products may, at its discretion, provide a replacement sample or refund any applicable shipping and handling fee.
Order Confirmation
After your sample order has been successfully submitted, you will receive an order confirmation by email.
Receipt of an order confirmation does not guarantee product availability. If Fortis Medical Products is unable to fulfill a requested sample, Fortis Medical Products reserves the right to substitute a requested sample with the most similar available product when the specific sample requested is unavailable.
Payment Terms
Fortismp.com strives to provide a secure and straightforward ordering experience. Any applicable shipping and handling fees are processed securely through our payment processor. Fortis Medical Products does not store complete credit card information on its servers. Payment is authorized when your order is placed. Sample orders will not be processed until any applicable payment has been successfully approved.
If your payment is declined, your sample order will not be completed. Please verify your payment information or contact your card issuer before attempting your purchase again.
Sales Tax
Applicable sales tax, if any, will be calculated at checkout based on your shipping address and applicable state and local tax laws.
Pricing
All prices displayed on fortismp.com are listed in U.S. Dollars (USD).
Fortis Medical Products reserves the right to change prices, shipping charges, or other fees without notice. The amount charged will be the amount displayed at the time your order is submitted.
Failed or Declined Payments
Orders associated with declined, expired, or otherwise unsuccessful payment methods will not be processed. Fortis Medical Products reserves the right to cancel any order for which payment cannot be verified.
Promotional Codes
If promotional discounts or coupon codes are offered on fortismp.com, they must be entered during checkout and cannot be applied after an order has been completed unless required by law. Promotions may not be combined unless specifically stated.

Fortismp.com uses industry-standard encryption and secure payment processing technologies to help safeguard payment information during transmission.
Fraud Prevention
To help protect our customers and Fortis Medical Products, we reserve the right to review and verify orders that appear unusual or present a potential risk of fraud.Fortis Medical Products may contact customers to confirm order details before an order is processed. Orders suspected of fraudulent activity may be delayed or canceled.
Medical Expense Eligibility
Fortismp.com does not currently process payments directly through Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA).
Sample Orders & Refund Policy
Fortis Medical Products is pleased to offer product samples so customers can evaluate Fortis Medical Products ostomy products before purchasing.
Because sample orders are provided at no charge for the products themselves, sample products are generally not eligible for return or exchange. Any shipping and handling fee associated with a sample order is also generally non-refundable once the order has shipped.
Sample Order Limitations
To help ensure that samples are available to as many customers as possible, Fortis Medical Products may limit the number of sample orders that an individual customer may place.
Fortis Medical Products reserves the right to limit, deny, or cancel sample requests that appear to be duplicate requests, excessive requests, fraudulent, or otherwise inconsistent with the intended purpose of the sample program.
Sample Order Eligibility
Samples are provided for product evaluation purposes. Fortis Medical Products reserves the right to determine sample availability and the appropriate quantity and type of samples provided.
If a specific sample requested is unavailable, Fortis Medical Products may provide the most similar available sample based on the information provided with the request.
Damaged or Defective Samples
If you receive a sample that is damaged or defective, please contact Fortis Medical Products Customer Care as soon as possible after delivery.
Please provide your name, order information, and a description of the issue. Photographs may be requested to help us investigate the issue. Depending on the circumstances, Fortis Medical Products may provide a replacement sample or refund any applicable shipping and handling fee.
Changes or Cancellations
If you need to change or cancel a sample request, please contact Fortis Medical Products Customer Care as soon as possible.
Because sample orders may be processed promptly, Fortis Medical Products cannot guarantee that a request can be changed or cancelled after processing or shipment.
Refunds
Because the products included in the Fortis Medical Products sample program are provided free of charge, there is generally no product purchase price to refund. If Fortis Medical Products determines that a refund is appropriate, any applicable refund will generally be limited to the shipping and handling fee paid for the affected sample order.
Approved refunds will be issued to the original payment method. Depending on your financial institution, additional time may be required for the credit to appear on your account.
Exchanges
Fortis Medical Products does not offer direct exchanges for sample products.
If you received an incorrect, damaged, or defective sample, please contact Customer Care. Fortis Medical Products may or may not provide a replacement sample at its discretion.
